Patient Services and Support

Patient Services at Erlanger Community Health Centers are provided through a process established to ensure the delivery of safe and timely care and treatment in an atmosphere of respect and dignity. Patient Services are planned, provided and supervised by professional healthcare providers working together for positive outcomes in your health.

The Community Health Center was started in 1968 with the establishment of the Alton Park Neighborhood Health Center (now Southside). In 1976 the Health Center expanded to the eastern portion of Chattanooga with the opening of another facility on Dodson Avenue. In 1977, the Community Health Center locations became a part of the Erlanger Health System. Lastly, Premier Health Care on Lyerly Street became the third Erlanger Community Health Center location.

The Community Health Center clinics provide comprehensive primary medical and dental care services, behavior health, disease management and wellness promotion services to all, focusing on the medically underserved and uninsured. The Community Health Centers accept Medicare, Medicaid, and commercial insurance. Care is provided to all persons without regard to ability to pay through the use of a sliding fee scale.
